Home health care plays an important role in a patients recovery. It allows
patients to receive needed health related services in the comfort of their own
home, surrounded by their family, friends and a supportive health care team. Our
multi-disciplinary team will focus on helping patients and their families become
independent in their care and remain independent in their home environment.
Home health care may be used to learn:
- About disease processes and medications
- How to manage ongoing health issues
- Home exercises to enhance ability to be independent and regain lost function
- How to accomplish activities of daily living
- How to address swallowing, language and cognitive problems
- How to do wound care, promote healing and prevent skin breakdown
At Carondelet Home Care Services, our team of professionals works under the
direction of the patients physician to provide a wide range of services
to meet the needs of each patient.
Our Services
A registered nurse or physical therapist makes a thorough assessment of the
patients needs during the initial home visit and develops a treatment
plan with the patient and physician.
Nursing
Our nursing staff provide on-going assessments to determine status of health
care issues and teach patients about their medications and how to be independent
in managing their health care concerns on a day to day basis. More specialized
areas of nursing include:
- Ostomy and Wound Care- A nurse with special training assists patients with ostomies or other skin problems. Patients and families receive extensive training in order to feel confident assuming this care themselves.
- Parenteral/Enteral Therapy- A highly trained registered nurse provides care and instruction to patients who need intravenous fluids, antibiotics or tube feedings.
Our nursing team is supported by licensed practical nurses and home health
aides who work under the supervision of the RN. They are available to assist
with bathing, dressing, skin car eand oral hygiene. Our LPNs will assist with
wound care, lab draws and other skilled nursing needs.
Rehabilitation
Our rehabilitation team is prepared to help patients reach their maximum level
of independence. After completion of a comprehensive evaluation, the therapist
will develop an appropriate program incorporating therapeutic exercises, activties
of daily living and physical modalities as indicated. Home safety as well as
family and patient education is an important component of our comprehensive
approach. Our team consists of::
- Physical therapists (PT) and physical therapist assistants (PTA)- Physical therapists utilize therapeutic exercises, gait training and physical modalities to help restore mobility, improve balance, range of motion and strength and decrease pain. Our PTs are especially experienced in the post-hospital rehabilitationof patients with joint replacements andprovide pre-operative evaluation visits to assess home safety and post-hospital equipment needs. They also have certified training in the management of lymphedema.
- Occupational therapists (OT) and certified occupational therapist assistant (COTA)- Ots assist patients to regain independence through functional and task based retraining and the use of adaptive equipment for daily living. They are experienced in energy conservation techniques to allow increased activity in the home while decreasing fatigue.
- Speech languate patholigist (SLP)- Speech language pathologists assist in the recover of the ability to communicate through spoke language. Cognitive retraining also enhances the patients ability to regaing home-based independence. Our SLPs have special training in the use of electrical stimulation to treat swallowing problems.
Social Work Services
A medical social worker can assist the patient and family with social and economic
problems resulting fro illness and acts as a liaison with community resource
agencies.
Coverage
Home health care services are reimbursable through Medicare, Missouri Medicaid,
workers compensation and many health insurance programs. Our staff can
assist in determing insurance coverage.
Medicare Eligibility
Requirements for Medicare eligibility include:
- Under the regular care of a physician
- Need intermittent, part-time, skilled nursing or therapy
- Meets criteria for homebound status

Medicare certified agency. Accredited by Joint Commission Accreditation of Healthcare
Organizations.

Carondelet Home Care Servcies provides services in the greater Kansas City area
including Jackson, Western Lafayette and Northern Cass counties in Missouri
and Johnson and North Miami counties in Kansas.
