Prior to Admission

Once you have chosen Carondelet Manor for you or your family member, contact the admissions coordinator so arrangements can be made. You will need to have a physician, licensed in the State of Missouri that will provide orders for medications, treatments, diet and activity who is able to make required visits to Carondelet Manor. Your medical history and physical is necessary from the physician within five days prior to admission or within two days following admission. If you do not have a physician, the admissions coordinator will assist you in obtaining a physician who accepts residents in the facility.

If you are coming from a hospital or another nursing facility, our staff will obtain this information from that facility.

Please contact the admission coordinator to schedule a time to complete all necessary admission paperwork and review any special needs. Ask about facility guidelines regarding bringing furniture, pictures or electrical appliances from home.

Information Needed Prior to Admission Day

  • Full name, birth date and place of birth
  • Name, address and telephone number for the following: the responsible party, the emergency contact person, the resident's dentist, pharmacy and mortuary.
  • Copies of advanced directives, health care proxies, living wills or durable powers of attorney.
  • Resident's Medicare card, Social Security card and insurance card(s). For medical records, we will also need the name, address and telephone number of the attending physician and/or other physicians who will be providing care.

What to Bring

You are encouraged to bring personal belongings and furnishings that will help you feel "close to home." Due to space and safety restrictions, items must be limited to personal clothing, furniture, books, photographs, electric shavers, televisions, radios, telephones, wall decorations and toiletries. Questions about what to bring may be discussed with the admission coordinator or social worker. Our social workers are available to help ease the transition for both you and your loved one and serve as an ongoing source of support as normal living situations occur.

Medications

Pharmacy fees are not included in the daily room rate. You are free to select any pharmacy that will bubble pack and deliver a 31-day supply of medicationm, delivered to our facility, to be in compliance with our system. Pharmacy bills are mailed monthly and are the responsibility of the resident or responsible party. Medications of any kind cannot be kept in your room, including over the counter medication unless there is a physicians order and the approval of the care plan team. This is for your safety as well as the safety and welfare of other residents.

Transportation

When a resident is admitted to our facility from a hospital or other health institution, the transportation is handled by the transferring facility.

If a resident is admitted from home, the transportation to Carondelet Manor is handled by the family or responsible party.

Once a resident is admitted to Carondelet Manor and requires transportation to a doctor's appointment or out-patient tests at a hospital/clinic, our social service workers will discuss transportation options and the cost for transportation with the resident and responsible party. There is a service charge that Medicare/managed care does not cover. This becomes the responsibility of the resident.

Payment/Refunds

An advance payment is required at the time of admission for private pay services. The payment due would be the daily room rate times the remaining days of the current month. The social worker will assist with the Medicare/Medicaid and managed care arrangements.

An advance payment is required at the time of transition, which occurs when a resident stays beyond the days covered by Medicare/managed care and then becomes a private pay resident.

When a resident leaves Carondelet Manor, any remaining balance will be refunded after the final charges have been cleared through the Business Office.

Visiting Hours

We recognize the importance of your family and friends. Our visiting hours are open and flexible to accommodate you and your family and friends needs. Our front entrance is open from 8 AM to 8 PM every day. After 8 PM, you will have to use the Subacute entrance located at the back of the building. We do have a receptionist available to assist you from 8 AM until 8 PM daily.

In order to enhance the quality of care of our residents, we do, however, ask that you be respectful of their home environment. Please maintain minimal noise levels, knock before entering any room, and please conduct your visit in a kind and pleasant manner.

We also encourage children and pets to visit our residents. We do request that prior to a visit from a pet that you contact our Social Worker, Activity Coordinator, or an administrative representative for specific instructions.

Telephones and Televisions

Telephone service and televisions are provided on the Subacute Unit. Otherwise, telephone and cable receptacles are located in other resident rooms in the facility. To arrange for these services, family members should contact the appropriate vendors for direct billing.
