Sub Acute
Care
When someone's medical condition is stable after
a hospital stay, yet they need additional time
to recuperate, subacute nursing care might be
their best option for continued care. This unit
provides more intensive treatment for patients
requiring specialty care.
Subacute care is intended to be a short-term path to recovery. Our subacute unit utilizes a very goal-oriented approach. For each patient, an individual treatment program is developed. Our experienced physicians, nurses and rehabilitation therapists work to bring each patient to his or her own predetermined treatment goals.
When those treatment goals are met, a patient is then ready to move on. Because Carondelet Manor offers such a comprehensive range of services, the patient can easily transition to home or to our long-term care unit.
Our sub acute unit provides:
- Care for individuals requiring rehabilitation
or who have nursing care needs related to medically
complex diagnoses.
- Piped in-wall oxygen in patient rooms and
dining room
- RN's on staff 24 hours per day, 7 days per
week
- Nurses with expertise in tracheotomies; I.V.'s
to include antibiotics, PICC lines, central
lines; wound care to include wound vacs, specialty
beds; pain management and more.
- Private and semi-private rooms

Therapy
Our speech-language pathologists assist in restoring
quality of life and independence with Vital
Stimulation Therapy.
Swallowing is a function so basic that most of us take it for granted. Yet each year thousands of people lose their independence and quality of life because of difficulty swallowing. The inability to swallow is called dysphagia. This condition is often caused by stroke, Parkinson's disease, multiple sclerosis, ALS (Lou Gehrig's disease), head injury or head and neck cancers. No matter what the cause, the result is often the same - the patient cannot swallow. Vital Stimulation Therapy is non-invasive and painless.

Lymphedema
Therapy
Carondelet Manor offers Complete Decongestive
Therapy (CDT) to individuals suffering from a
common chronic condition affecting the lymphatic
system, known as lymphedema. CDT treatments improve
the individual's functional outcome and quality
of life.
Lymphedema is an accumulation of lymphatic fluid that causes abnormal swelling, usually of the arms or legs. The condition often develops after lymph nodes are removed during cancer surgery, for example; or when lymphatic vessels are impaired or damaged due to illness or trauma.
Individuals with mild to moderate lymphedema can be successfully treated with Complete Decongestive Therapy (CDT).

Care
A Personalized Approach
Carondelet Manor's wound care program is designed
to provide the latest and best medical treatment
of wounds-specific to the individual. We focus
on assisting the patient who has had to alter
his or her lifestyle due to a recurring or slow-to-heal
wound. Our goal is to help wounds heal quickly,
ultimately allowing the patient greater comfort
and improved quality of life.
We keep the patient fully informed during every phase of the treatment plan, making sure he or she understands the treatment and the reasons behind it. We encourage the patient and his or her family to participate in discussions about the treatment plan. We have found this approach enhances the healing process.
The nurses at Carondelet Manor have expertise in wound care including wound vacs. We utilize an interdisciplinary team approach in dealing with the treatment of wounds. The team includes dietitians, nurses, physical therapists, wound care consultants and physicians. The wound care team can provide many services including specialty beds and long-term IV antibiotic therapy.

to the care of individuals with Alzheimer's disease

for the primary caregiver by admitting their home
care patient to our facility.

Nursing
While the size of staff varies with the number of patients and their needs, Carondelet Manor strives for a very high staff-to-patient ratio - higher than those mandated by the State of Missouri. We provide staffing based on patient acuity levels.
Not only do we employ more caregivers - but we employ those who care more - only competent professionals who exhibit attitudes of caring and compassion. We value our employees and strive to retain the best. Staff credentials and training are matched to patient needs and acuity levels. The stability of our staff means that patients and staff get to know each other. This means improved efficiency and patient comfort.
Professional and licensed nurses are experts in
the care of the older adult. Our nurses provide
care, including wound and ostomy care, pain management,
IV therapy, enteral feeding, catheter care and
more.
Certified nursing assistants provide patients
with assistance in activities of daily living
including bathing, dressing, grooming, toileting,
transfer and ambulation.
Restorative nursing assistants, specially trained
and certified in rehabilitation techniques, assist
our rehabilitation staff and our patients to attain
and/or maintain their optimal level of well-being.
A registered nurse coordinates the interdisciplinary
team to develop a written individualized plan
of care structured specifically to the unique
needs of each individual.
Our staff development coordinator works with
new nursing employees in orientation on the expectations
of care, competencies and technical skills required.
The nursing staff receives ongoing education and
annual review of their competencies.

Rehabilitation
We provide a full spectrum of on-site rehabilitation
services in physical, occupational and speech-language
pathology.
Physical therapy helps restore mobility
and prevents further disability and enables participants
to regain independence and boost self-esteem.
Exercises play a critical role in strengthening,
endurance, pain and balance control.
Occupational therapy helps individuals
regain the ability to manage activities of daily
life (dressing, grooming, eating, bathing, meal
preparation, laundry, grocery shopping, etc.).
Speech-language pathologists (i.e. speech
therapists) help individuals with verbal and
non-verbal communication skills plus minimize
swallowing difficulty that may accompany an injury
or illness.
Our therapists guide patients and families through
the rehabilitation process developing customized
treatment plans to maintain the highest level
of independence possible once maximum potential
is reached. After a fall, surgery, stroke or debilitating
illness, it's this personalized approach that
helps individuals get their lives back on track.

Social Services
The social workers at Carondelet Manor are an
integral part of the health care team. These individuals
serve as advocates for the patient, family and
significant others; provide assistance with admission
and discharge planning; and promote an optimal
level of physical, social and psychosocial well-being.
Social Services work with patients and families
on planning for patients who are being discharged
to home. They provide resources, as well as make
contact with home health companies and other resources
as is necessary to make the patients home going
a success.

Dietary
Services
Carondelet Manor makes every effort to ensure
that mealtimes are a pleasant experience. The
registered dietician and the certified dietary
manager plan meals and therapeutic diets to meet
the needs of the patients.
Three patient dining rooms are available and
are supervised by licensed nursing staff. Private
dining facilities, scheduled upon request, allow
patients to share special mealtimes with friends
and family. Guest meals are served for a nominal
fee.

Activities
At Carondelet Manor, we feel that being active
and having fun are important keys to staying healthy.
Our staff activities director organizes activities
for individuals and groups to meet their spiritual,
physical, social, intellectual and creative needs.
The activities department meets
with the patient to determine what they personally
enjoy and to encourages them to participate with
groups to get to know other patients.

Spiritual
Care
Healing of the body, mind and spirit is a hallmark
of the care received throughout Carondelet Health
system. The spirit of an individual is basic to life and identity. It is our goal to honor and nuture the Spiritual Path of each person.
The Spiritual Care staff is composed of men and women, volunteers and students; both lay and ordained, who represent a variety of faiths.
Spiritual Care includes weekly Mass, daily Communion, Anointing of the sick, interdenominational worship services, Bible study, prayer srevices, pastoral counseling, Advanced Directives for Health Care Decisions, Durable Power of Attorney
for health Care and Spiritual support.

Physician
The medical director of Carondelet Manor is the
key to maintaining excellent medical services.
The medical director provides leadership in developing
and implementing policies and procedures and serves
as a liaison between primary care physicians and
the health care team.

Convenience Services

Laundry
Patients may choose to have personal belongings
laundered at the facility or sent home with family
members. All bed linens and towels are routinely
laundered on the premises.
|
|
Beautician
Appointments can be arranged with the hair stylists
Monday through Friday for haircuts, permanents,
shampoo and sets, manicures and pedicures. The
cost of these services are not included in the
daily rate. Our stylists are all licensed cosmetologists.
|
|
Transportation
A wheel chair accessible van is available to transport
residents to St. Joseph Medical Center or on-campus
medical buildings for a nominal fee.
|
|
Maintenance
Carondelet Manor's maintenance department will
assist to make your stay here a comfortable one.
Our maintenance department is available to assist
with hanging patient pictures and photos and to
assist patient and families with use of cable
television.

Housekeeping
Our friendly housekeeping staff provides a clean
environment for our patients. They ensure the
rooms are ready for new patients coming to Carondelet
Manor.
