Call 816-943-4799 to schedule an appointment for diagnostic testing at St. Joseph Medical Center,
Monday through Friday, 8 a.m. to 5:30 p.m.
A physician's signed written order is required for all diagnostic testing, except dietary consult, pain management consult or yearly mammogram screening. The physician's order can either be faxed to 816-943-4560 or the patient may bring the order at the time of service.
An order should contain the following information:
- Date
- Patient's Name (DOB is very helpful also)
- Procedure to be done
- Diagnosis
- Physician signature
