Diabetes can be controlled. It takes some lifestyle
changes and support from loved ones, but it can be done. At St.
Joseph Medical Center and St. Mary's Medical Center have been recognized by the American Diabetes Association for Quality Self-Management Education. We
offer several programs to help make living with diabetes easier.
Inpatient Services
Carondelet Health is committed to supporting the patient with diabetes. We provide excellent nursing care and individualized diabetes education is available to any hospitalized patient. We are dedicated to sending our patients home with the knowledge they need to manage their diabetes on a day-to-day basis.
Carondelet Health has nursing units that specialize in diabetes.
Nurses on the units have extensive training in diabetes care. We
offer excellent nursing care and individualized diabetes education
to our hospitalized patients. We are committed to sending our patients
home with the knowledge needed to manage diabetes on a day-to-day
basis.
Diabetes Management Course
St. Joseph Medical Center and St. Mary's Medical Center offer diabetes
education on an outpatient basis, too. The Diabetes Management Course
is offered each month to teach the participants basic management
techniques. It is recommended for people who are newly diagnosed
or those who have not had a formal class in the past five years.
Attendance by family members or loved ones is encouraged. The course
is offered at a variety of times to accommodate most schedules. Diabetes Management Course Schedule.
In addition to the class sessions, the Diabetes
Management Course includes a one-on-one session with a dietitian and
a pre-class assessment with a Diabetes Nurse Educator.
The Nurse Educator provides a follow-up phone call at around three months after completion of the class.
Support/Information Sessions
We offer three monthly Diabetes Support Groups to educate as well as address issues and concerns for the patient with Type 1 or Type 2 Diabetes. Support Groups are free and open to the public. They are held at a variety of daytime and evening hours. We encourage patients to include their family and friends as a part of these monthly meetings. We also regularly host other educational events of interest to the patient with diabetes.
Individual Education
Individualized education and counseling with a professional is invaluable
when you need assistance in solving long-term problems. One-on-one
education sessions are available with registered dietitians and
a certified diabetes educator and/or registered nurse.
