It is the mission of St. Mary’s Rehabilitation Center to work with each patient and their family to regain maximum independence. Through a comprehensive, interdisciplinary program, St. Mary’s Rehab Center works to achieve mutually agreed upon goals to return each patient to the highest level of functioning in body, mind, and spirit.

About Our Program
- Opened in 1989
- 15 licensed inpatient rehab beds
- Only inpatient rehab unit in eight counties between Blue Springs and Columbia, Missouri
- Routine updates provided to the patient's primary care physician
- Discharge summaries sent to the patient's primary care physician
- Follow-up patient appointments with the primary care physician
- 24/7 medical direction provided by a licensed physiatrist provided
- 24/7 rehabilitation nursing
- Occupational therapy, physical therapy, speech/language pathology, social work/case management, dietitian, and psychiatry
- Weekly interdisciplinary team conferences with patient and family
- Preauthorization and on-going communication with insurance companies
- Peer visitation program for stroke patients
- Active support group for stroke survivors post discharge
A Focus on Patients and Their Families
Core to all rehabilitation efforts is the preservation of the family unit. The staff of the rehabilitation center works with the patient and family to help the patient return home. Toward that goal, the rehab team will:
- Establish individualized programming designed to aid each patient in achieving optimal level of functioning
- Include the patient and family in establishing realistic and functional goals
- Provide on-going family training
- Complete home assessments, when needed, to provide modification recommendations for increased safety and independence upon return home
- Allow patients and their families to practice newly acquired skills by using the independent living apartment on the unit prior to discharge
- Partner with other providers to assure the patient receives the needed level of care after discharge from the unit
- Provide opportunities for reintegration into the community, when appropriate, by taking patients on therapeutic outings in our wheelchair accessible van
Admission Criteria
Services are provided for persons age 18 and older. Admission criteria include:
- Medical necessity
- Motivation and ability to participate in therapies at a minimum of three hours at least five days per week
- An identified discharge plan
- A need for rehabilitation nursing and medical care 24 hours a day
- Potential for functional improvement
- No recent completion of an inpatient rehabilitation program for the same injury or illness, unless a change in condition has occurred and is documented

- Stroke (CVA)
- Brain injury
- Spinal cord injury
- Amputation
- Hip fracture
- Major multiple trauma
- Burns
- Active polyarticular rheumatoid arthritis, psoriatic arthritis and seronegative arthropathies
- Systemic vasculidities with joint inflammation
- Severe or advanced osteoarthritis
- Multiple Sclerosis
- Parkinson's Disease
- Guillain Barre'
- Polyneuropathy
- Other progressive neurological impairments
