Upon completion of a confidential online health risk assessment, your employee will receive a Personal Wellness Profile which identifies areas of risk, provides specific recommendations for improving health, and can track yearly health changes. An aggregate Executive Summary provides information about the overall health of your employees. This information can then be used to direct and evaluate your wellness program. Paper assessments and additional reports are also available.
How the Personal Wellness Profile Online Assessment Works:
For the Employee
The user completes a 40 question online assessment designed to evaluate their current state of health and wellness. Since your employee links directly to the Carondelet Health website, they are assured that all health information is secured and confidential.
Results are tabulated in real time to create a report summary that compares your employee's scores against an optimum score gleaned from the latest health research guidelines.
From the report summary, the user can review the categories where improvements need to be made and then click through to the Online Wellness Library, which provides prevention and wellness articles that address specific topics. Each article contains a list of links that point to related articles within the library. All health information is based on scientific studies and reports from the latest health industry literature.
For the Employer
A group Summary Report is available as a custom option. This report provides your organization with key information important for planning wellness programs and measuring the success of your program. An Executive Summary includes a scientific estimate of the cost benefit from employee lifestyle changes.
