About Carondelet Home Care Services

Carondelet Home Care Services is a full service home health agency. Our multi-disciplinary team focuses on helping patients and their families become independent in their care and remain independent in their home environment. At Carondelet Home Care Services, our team of professionals works under the direction of the patient’s physician to provide a wide range of services to meet the needs of each patient.

Carondelet Home Care Services is affiliated with Carondelet Health, Kansas City and members of Ascension Health, the nation’s largest Catholic non-profit health care organization. We are committed to our Mission, Vision and Values of providing compassionate, quality health care for body, mind and spirit.

Carondelet Home Care Services is a Medicare certified home health agency licensed in Missouri and Kansas and Joint Commission accredited. We provide home health services including:

• Physical therapy
• Occupational therapy
• Speech therapy
• Medical social work
• Home health aides
