Professional Practice Model Development
The professional practice model at CH provides a true depiction of how our nurses provide care, including our centralized focal areas: patients, patients families and communities. This framework is supported by Swanson’s Caring Theory. It highlights the five major supporting concepts: Knowing, Doing for, Being with, Empowering and Maintaining Belief. This structural framework allows our nursing staff to develop personalized relationships when caring for the populations we serve.
The major tenets of the professional practice model have been in place since 2007. However, a schematic that represents our model needed further development. The beginning of the project started in late 2011 and was completed in early 2012. Our patient care council and professional development council played the primary role in developing how the model should look, the colors that should be included, and which pieces should be extensions of our center point (patients, families and communities).
Our nurses feel that our nursing theoretical framework supports the essence of our care delivery. Therefore, the larger, more visible extensions from the center circle highlight Swanson’s major concepts. The other extensions are slightly smaller and incorporate the other major tenets of professional practice. We discussed which overarching topics should be included and how the model should look as well as the colors in the model. The team felt that the colors should mimic the organizational color scheme. They stated this was important to show that the nursing division is part of the health care system in the organization and not separate, showing collaboration with other disciplines.
